Clinical Management of Non-metastatic and Metastatic Trophoblastic Disease


Abstract
Gestational trophoblastic neoplasm¡¯s are recently one of the most curable gynecological malignancies and the characteristics of gestational trophoblastic neoplasm¡¯s are more than in young women, its propensity for early metastasis, its gonadotropin tumor marker, and its rapid usual response to chemotherapy.
Recently, non-metastatic and low risk metastatic gestational trophoblastic neoplasm patients should have a remission rate close to 100% by primary single chemotherapy, multi drug regimen (MAC and CHAMOCA), and surgical procedures were useful only to remove the confined focus of disease, to control hemorrhage, to relieve obstruction, to treat infection, and to treat other complications. But high risk metastatic trophoblastic disease has very high recurrence rates(21%) and low remission rates(60%) by multi agent chemotherapy with various irradiation. Therefore we should be try to manage high risk metastatic trophoblastic disease by intensive therapy and more multidiscriptive approach.
